The Maxillo3D Clinic must collect the personal information necessary for the provision of dental care using the attached confidential medical questionnaire which is presented to you for signature.<\/p>\n

The confidential medical dental questionnaire is kept in your dental file, and your dental file is kept in the Maxillo3D clinic to which the public does not have access.<\/p>\n
