Personal information collected

The Maxillo3D Clinic must collect the personal information necessary for the provision of dental care using the attached confidential medical questionnaire which is presented to you for signature.

The confidential medical dental questionnaire is kept in your dental file, and your dental file is kept in the Maxillo3D clinic to which the public does not have access.

Or

The confidential medical dental questionnaire is kept in your dental file, and your dental file is kept on digital media, access to which is controlled using access codes.

Only healthcare staff will have access to your health information, administrative staff will only have access to the information necessary to carry out their duties (billing, making appointments, transmission to insurance companies).

Duration of retention of personal information.

Your dental file is kept for five years following the last registration or insertion in this file in accordance with the law and at the end of this period, it will be destroyed in order to preserve its confidentiality.
